Deed Poll/ Change of Name Deed

A deed poll, also known as a change of name deed, is a crucial legal document that substantiates a name change. It's a self-declaration where you renounce your old name and adopt a new one. This document allows you to modify any part of your name, add or remove names and hyphens, or alter the spelling. 

How do you change the name on your Hong Kong identity card or passport?

If you wish to change your name on your Hong Kong identity card, follow these steps: First, prepare the deed poll. Then, complete the application form (ROP73) and send it by post to the Registration of Persons (Support) Section, 12/F, Immigration Tower, 7 Gloucester Road, Wan Chai, Hong Kong, along with a copy of the deed poll. Alternatively, you can submit the application form at any Registration of Persons Office (except Kwun Tong (Temporary) Office), by fax, through the Internet, or via the Immigration Mobile Application. Once your application is received, you'll be asked to attend an interview to present the originals of your supporting documents. If your application is approved, arrangements will be made to replace your identity card for a fee of HK$460. Once your new identity card is issued, you can use it to change the name on your HKSAR passport.

How do I change the surname of a legitimate child?

The child's surname is not on the Hong Kong birth certificate. Only the child's given name will be registered on the birth record, and the surname will be adopted from the father if the father's personal information is registered on the child's birth certificate. To change the surname of a legitimate child, a deed poll showing both the father's and mother's signature and consent is necessary. Even if the parents are divorced, and sole custody is granted to one father or the mother, the consent of both parents is required to change the surname of a legitimate child.

How do you change the surname of an illegitimate child?

If the mother wishes to change the surname of her illegitimate child with her declaration of illegitimacy, a deed poll showing the mother's signature and consent is necessary. There is no need to obtain permission from the child's father beforehand, provided no court order is in effect.
